Proposed Closure of Valley Surgery in December 2017 / ARGYMHELION I GAU MEDDYGFA Y FALI YN RHAGFYR 2017

PROPOSED CLOSURE OF VALLEY BRANCH SURGERY IN DECEMBER 2017 It is with great sadness that the partners have been forced to prepare for the closure of Valley Branch Surgery in December 2017. The effect on those patients that use this Branch Surgery will be an inconvenience and possibly some difficulty in accessing health care in the future which we deeply regret. Unfortunately, we are losing a Partner in August 2017 and despite our best efforts have failed to recruit a replacement GP to take over the role. We already have two Advanced Nurse Practitioners working in Holyhead and we are actively seeking another to join our team but this has also proven difficult. We are spread over three sites at Holyhead, Valley and Bodedern at present and we have a large number of patients which makes for a very busy Practice. When the Practice reduces from five to four Doctors for the first time for at least 40 years, the continuation of providing a service at three sites will be impossible and this is the reason for the planned closure. 10% of our patients are registered at Valley but a large number of those still use Bodedern and Holyhead for appointments. We realise that not having a Branch Surgery at Valley will be very inconvenient for those who use it but we will continue to be open from 8.00- 6.30 five days a week in Holyhead and will be increasing our service at Bodedern from three whole and two half days to five whole days at Bodedern to compensate. In addition, we are increasing the number of consulting rooms at Bodedern so that three clinicians can work at Bodedern at once rather than the current two. There are currently good transport links between Valley and Holyhead with both a bus and train service. It is with great regret that we have made this decision and we hope that the provision of the extra sessions at Bodedern will enable us to continue providing a high standard of Primary Care. You do not have to take any action to keep receiving health care from Victoria Surgery but have the right to register with another practice should you wish. This decision has been arrived at by discussions with the Local Health Board and with the Community Health Council. Valley Pharmacy, who own the current surgery building at Valley will continue to operate and provide the excellent service that they do. We personally feel sad having to make this decision but events have forced us into this corner and we cannot see any other way of keeping the quality of service to you, our patients. The North Wales Community Health Council are happy to provide information and support should you wish to contact them in confidence. This is an independent health watchdog representing the interests of patient and the public in North Wales and are contactable by either email Mrs Carol Williams, Deputy Chief Officer at Carol.Williams@wales.chc.org.uk, by telephoning Mrs Williams on 01248 679284 (ext 3) or writing to Mrs Carol Williams at North Wales Community Health Council, 11 Chestnut Court, Parc Menai, Bangor LL57 4FH. ______________________________________________________________ Gyda thristwch mawr, rydym fel partneriaid yn cael ein gorfodi i baratoi ar gyfer cau meddygfa y Fali yn Rhagfyr 2017. Bydd hyn yn peri anghyfleustra, ac efellai anhawster i gleifion fynedu gofal iechyd yn y dyfodol, ac mae hyn yn rhywbeth sy'n peri gofid i ni. Yn anffodus, byddwn yn colli un o'r partneriaid yn Awst 2017, ac er ein hymdrechion gorau, rydym wedi methu cael meddyg teulu i lenwi'r bwlch. Mae gennym eisioes ddwy uwch-ymarferydd nyrsio yn gweithio yng Nhgaergybi, a rydym yn brysur chwilio am un arall i ymuno a'r tim ond mae hyn yn profi'n anodd. Ar hyn o bryd, rydym wedi ein gwasgaru dros dri safle yng Nghaergybi, y Fali a Bodedern ac mae gennym nifer uchel o gleifion sy'n ei gwneud yn bractis prysur iawn. Bydd nifer y doctoriaid yn lleihau o bump i bedwar am y tro cyntaf ers 40 mlynedd sy'n golygu y bydd parhau i ddarparu gwasanaeth dros dri safle yn amhosib, a dyma’r prif rheswm dros gau. Yn bresennol, mae 10% o'n cleifion wedi eu cofrestru yn y Fali, ond gyda llawer iawn o'r rhain yn defnyddio Bodedern a Chaergybi am apwyntiadau. Rydym yn rhagweld y bydd cau cangen yn Fali yn anghyfleus i'r rhai sydd yn ei defnyddio, ond byddwn yn parhau i agor bum diwrnod yr wythnos o 08.00 - 18.30 yng Nghaergybi. Byddwn hefyd yn cynyddu'r gwasanaeth ym Modedern o dri diwrnod cyfan, a dau hanner diwrnod i bump diwrnod cyfan, er mwyn llenwi'r bwlch. Hefyd, byddwn yn cynyddu'r ystafelloedd ymgynghori fel y gall tri, yn hytrach na dau glinigydd weithio cyfochor. Byddwn yn parhau i gynnig gwasanaeth lawn yng Nghaergybi a Bodedern. Mae cysylltiadau trafnidiaeth cyhoeddus, bws a rheilffordd , aml rhwng y Fali a Chaergybi. Gyda gwir ofid y gwnaethwyd y penderfyniad hwn, ac rydym yn gobeithio y bydd y sesiynnau ychwanegol ym Modedern yn ein galluogi i barhau i gynnig gofal iechyd o safon uchel. Does dim rheidrwydd arnoch i weithredu mewn unrhyw fodd er mwyn parhau i dderbyn gofal gan feddygfa Victoria, ond mae gennych bob hawl i gofrestru gyda meddygfa arall os y dymunwch. Gwnaethwyd y penderfyniad yn dilyn trafodaethau gyda'r Bwrdd Iechyd Lleol a'r Cyngor Iechyd Cymunedol. Bydd fferyllfa y Fali sy’n berchen yr adeilad yn y Fali yn parhau i weithredu a chynnig gwasanaeth ardderchog. Yn bersonnol, teimlwn yn drist am y penderfyniad. ond mae digwyddiadau wedi ein gwthio i gornel ac ni allwn weld ffordd arall o gynnal safon y gwasanaeth i chi, ein cleifion. Mae Cyngor Meddygol Cymunedol Gogledd Cymru yn hapus i ddarparu gwybodaeth a chefnogaeth pe baech yn dymuno cysylltu a nhw yn gyfrinachol. Mae hwn yn gorff annibynol sy'n gwarchod a chynrychioli lles cleifion a'r cyhoedd yng Ngogledd Cymru. Mae posib cysylltu â hwy drwy ebostio Mrs Carol Williams, Dirprwy Brif Swyddog ar Carol.Williams@wales.chc.org.uk, ffonio 01248679284 (est 3) neu ysgrifennu at Mrs Carol Williams, Cyngor Iechyd Cymunedol Gogledd Cymru, 11 Chestnut court, Parc Menai, Bangor LL57 4FH.
